After August 1, 2009 = $325
Prices above includes the $60 Volunteer
Deposit,
refundable after completing 10 volunteer hours by 12/31/09

Equipment/Uniforms Deposit at time of fittings,
refundable upon turning in equipment/uniforms at the end of season.

DAILY PRACTICES Aug. 1 - 31, 2009
at Harvey West Park in Santa Cruz
All Teams - Football and Cheer are practicing Monday thru Friday in the
month of August. Practices are generally 5:30PM to 7:30PM unless dictated
otherwise from the Head Coach.
Before any participant can BEGIN practice we must receive the
following:

Birth Certificate or copy of Passport page

SIGNED National Pop Warner Player Contract (given at PARENT MEETING
TBA)

Full year School Report Card for school year 2008 / 2009

Signed and completed National Pop Warner 2009 Player Medical Form
-
Head shot Photo (minimum size 2" x 3") - please print child's name on
back of photo

And All Fees paid in full

MANDATORY PHYSICAL EXAMS
All
participants must have a COMPLETED 2009 MEDICAL FORM.

from previous years.

Medical Examination at Santa Cruz Rehab Center on
WED. JUNE 24, 2009
This is our
annual Fundraiser
with 100% of the proceeds
goes to supporting the program.
$20 Physical
Exams (no medical
insurance necessary)
Begins
at 6:30PM at the Santa Cruz Medical Rehab Center

The
form must be completed on both sides and signed by the
child's parent and physician completing the physical exam.
All forms must include the medical office address & phone number.
