Registration Fees:
- $185 through May 10, 2008
- $225 May 11 through June 1, 2008
- $285 June 2 through July 31, 2008
- $325 August 1, 2008 and after
CHEER
Registration Fees:
- $150 through May 10, 2008
- $185 May 11 through June 1, 2008
- $215 June 2, 2008 through July 31,2008
- $250 August 1, 2008 and after

MANDATORY PHYSICAL EXAMS

All participants must have a COMPLETED MEDICAL FORM.
Santa Cruz Pop Warner will be hosting PHYSICAL EXAMS on Saturday, May 31st from 12 Noon to 3pm.
Current 2008 Pop Warner Physical Fitness & Medical History Form will be provided at the exams.

Exams will be at the Santa Cruz Medical Rehabilitation Clinic
1529 Seabright Avenue - Santa Cruz
The cost is $20, and all proceeds are returned to
Santa Cruz Pop Warner as our annual fundraiser.
